How to Fill Out the Accident Report
At the Accident Scene

General Instructions
- Use one sentence for two involved vehicles per report (two sentences for three vehicles, etc.).
- It doesn't matter who provides or fills out the form. Use a ballpoint pen and write firmly so that the copy is legible.

When Filling Out the Accident Report, Please Note:
- Answer the questions:
- a) according to item 8, using the information from your insurance documents (policy or Green Card)
- b) according to item 9, using the information from your driver's license
- Indicate the exact point of collision (item 10).
- Mark the relevant circumstances (1-17) of your accident (item 12) and note the number of marked fields at the end.
- Draw a sketch of the accident (item 13).
- Answer the questions:

Witnesses
- Provide the name of a witness to the accident, especially in case of disagreements.

Signatures
- Sign the accident report and have the other driver sign it as well. Each person should keep a copy of the form.
At Home
- Indicate where and when your vehicle can be inspected by an expert (item 14).
- Do not alter the accident report under any circumstances.
- Send the form to your insurance company immediately.
Special Cases
- If the other party involved in the accident has the same form (approved by the Comité Européen des Assurances) in a different language, remember that the forms are identical and the questions have the same content. The questions are numbered for this purpose.
- The form is also valid for accidents without third-party damage, such as with comprehensive insurance.
